Complex Disease Management Plan (CDMP) / Enhanced Primary Care (EPC)
Performance Podiatry treats patients referred by their GP under the Complex Disease Management Plan (CDMP) / Enhanced Primary Care (EPC) Plan.

If you have a current care plan in place, you are entitled to receive a rebate from Medicare for up to 5 visits per calendar year to Allied Health Professionals. This rebate is approximately equivalent to $55.00. If all allocated visits are not used within one calendar year, this can flow over and still be valid in the following year.

Performance Podiatry is a private billing practice and requires full payment for consultations to be made up front. We encourage you to bring in your Medicare card and Debit card to allow us to perform an on the spot rebate with our HICAPS machine for eligible patients.

To be eligible for the Complex Disease Management Plan, your General Practitioner must assist you in managing chronic medical conditions such as

  • Arthritis
  • Asthma
  • Diabetes
  • Cancer
  • Heart Disease
  • Mental Health Illness

After both your initial appointment and final appointment on the referral, your Podiatrist will send an in-depth letter to the referring doctor outlining all assessment results, treatment plans and management strategies implemented to assist in maintaining optimal foot and leg health.
